Academic Accommodations

  1. Request for New Academic Accommodations

    2023-2024

    This request is for students of any class standing (including admitted new students and graduate students) who have not had accommodations in their classes before at Mount Union. Submission of this form is your official request for academic accommodations in your coursework for the 2023-2024 academic year and registration with the Office of Student Accessibility Services (SAS). Academic accommodations are unique and reasonable allowances based upon a student’s short or long-term disability, mobility, or health needs. Separate documentation must also be submitted.

  2. Renewal of Academic Accommodations

    2023-2024

    For returning Mount Union students (sophomores and higher) who have had academic accommodations in a prior academic year and wish to renew their accommodations for the 2023-2024 academic year.

  3. Request to Schedule Exam/Quiz

    2023-2024

    If you are a student already approved for testing accommodations throughout the 2023-2024 academic year, you can use this form to request to schedule an exam/quiz. Submission of this form is a request to indicate you would like to utilize your accommodations and will be taking an exam/quiz in the testing center. Your exam/quiz will be officially scheduled by Student Accessibility Services and you will receive confirmation on your exam/quiz.

    Exams/quizzes need to be requested by 5 p.m., at least three business days prior to the exam/quiz date.

Housing Accommodations

  1. Request for Housing and Dining Accommodations

    2023-2024

    For Mount Union students of any class standing who wish to request accommodations in their campus housing and dining plans for the 2023-2024 academic year. Submission of this form is your official request for housing accommodations for the 2023-2024 academic year and registration with the Office of Student Accessibility Services. Housing and dining accommodations are unique and reasonable allowances or exemptions based upon a student’s short or long-term disability, mobility, or health needs. Separate documentation must also be submitted.

Informed Consent

(Release of Information for Services for Students with Disabilities)

Confidentiality of information is an important component of the work done in the Office of Student Accessibility Services (SAS). Information provided to SAS is considered part of your educational record and is covered under the Federal Family Education Rights and Privacy Act (FERPA).

Information provided to this office will be used to establish ADA/504 eligibility and to develop appropriate accommodations for a given disability. At times it is necessary to speak with professors and other University personnel regarding a SAS registered student. University officials who have a legitimate educational interest in your disability status also have access to FERPA protected records.

For SAS purposes, this release of information authorizes the exchange of information between SAS officials and your current and past professors, any agency or person who conducted an evaluation submitted for consideration of accommodations, and qualified staff of the University.

SAS encourages students to obtain and keep copies of their documentation for future use. SAS will not forward documentation that originated with another institution or professional, nor will we provide copies for your records.

If you have any questions regarding these policies, please discuss them with the Office of Student Accessibility Services or Center for Student Success before signing this release.
